Death certificates are available for persons whose death occurred from 1891 - present in Hamilton County, OUTSIDE the city limits of Cincinnati or Norwood.

How to request a certified copy of a death certificate

Certified copies of death records are $19.00 and may be obtained in person, by mail or online (additional fee for this service). 

By Mail
Send the following items to the Hamilton County Public Health, 250 William Howard Taft Road, 2nd Floor, Cincinnati, Ohio 45219: 

  1. Completed application (see Dowload at right). 
  2. Check or money order (no cash) payable to the Hamilton County Public Health. Certified copies of death records are $19.00 each. 
  3. Self-addressed, stamped, business sized return envelope.

In Person
Bring the following items to the Hamilton County Public Health (directions): 

  1. Payment - certified copies of death records are $19.00 each. Cash, checks, money orders and debit cards ($2.50 processing fee applies) are accepted at the walk up counter.

Request a copy of a certified death record online using VitalChek. There is an additional fee to use this service, but you can choose to receive your certificate in as few as 2-3 business days.


Veteran's Death Certificates

The family of a veteran is entitled to one free copy of that veteran's death certificate. This certificate is marked "For Government Use" and can only be used to process veteran affairs. A VA application must be submitted by either the funeral home or the Veteran's Administration office. For more information, please call (513) 946-7805.


Certificate Corrections

Once a death/birth record is filed, an affidavit is required to change or correct any information on the certificate. The original document will not change - the attached affidavit will indicate the change/correction. An individual must go through Hamilton County Probate Court to make the change or correction to the original record.
